Predictors of Quality of Life in Subacute Stage of Stroke

Abstract

Objectives : This study aimed to investigate the factors predicting the quality of life (QOL) of stroke survivors in the subacute stage.
Methods : A total of 423 patients were evaluated within two weeks of stroke and 151(35.7%) were reevaluated after three months of the index event. Patients¡¯ QOL were assessed by World Health Organization Quality of Life-Abbreviated form (WHOQOL-BREF) which is composed of physical, psychological, social relationship and environmental domains. Concurrently, stroke severity, physical disability, cognitive function, grip strength and psychological distress (Hospital Anxiety and Depression Scale depression and anxiety subscale: HADS-D and HADS-A) were assessed as well as socio-demographic characteristics. The longitudinal associations of baseline variables were examined using linear regression analysis.
Results : Lesser physical disability and stronger hand grip strength predicted higher physical domain of WHOQOL-BREF (all p£¼0.05). Additionally, lesser physical disability predicted higher psychological (p£¼0.01) and social interaction (p£¼0.05) domain. Higher education level (p£¼0.001) and lesser stroke severity (p£¼0.01) predicted higher environmental domain. Higher HADS-D score predicted higher overall domains (physical p£¼0.001 ; psychological p£¼0.001 ; social interaction p£¼0.001 ; environmental p£¼0.01).
Conclusion : Findings from this study indicate the importance of management of depressive symptoms during the acute stage of stroke. Rehabilitation programs targeting hand strength and management of susceptible groups such as physically disabled or those with lower education level would also be important for improvement of QOL during the subacute recovery period in stroke survivors.
